Job Summary
To support a leading national healthcare organization, the remote Medication Therapy Management (MTM) Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and enhance patient education and adherence while working on a contract basis.
Key responsibilities
- Conduct telephonic consultations with members to complete comprehensive medication reviews and adherence assessments
- Identify gaps in care, high-risk medications, and therapeutic alternatives while evaluating medication regimens
- Provide patient education regarding medications and treatment plans, and communicate clinical recommendations to members and healthcare providers
Required qualifications
- Active, unrestricted Pharmacist license in your state of residence
- Strong understanding of disease-state management and the five core elements of Medication Therapy Management (MTM)
- Ability to navigate multiple clinical platforms, databases, and applications simultaneously
- Intermediate proficiency with Microsoft Word, Excel, and Outlook
- Flexibility to work rotating shifts between 9:00 AM and 8:00 PM CST, Monday-Friday
