Job Summary
To support a growing virtual Palliative Care program, the full-time State Licensed Palliative Care Provider (NP or PA) will manage patient assessments, develop Palliative Plans of Care, and provide consultative visits while working remotely across multiple states.
Key responsibilities
- Perform initial assessments and develop Palliative Plans of Care for patients enrolling in the program
- Conduct regular telephonic and/or video encounters with high-risk patients to manage their care
- Collaborate with interprofessional teams to coordinate seamless care and support transitions of care for patients
Required qualifications
- Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state
- Minimum of 5 years in advanced practice, with at least 3 years in Palliative care
- ACHPN credential or willingness to obtain within the first six months of employment
- Ability to flex between time zones and work effectively in a fast-paced, remote environment
- Strong interpersonal skills and comfort working within an interprofessional care team
