Job Summary
To support kidney health management initiatives, the part-time State Licensed Pharmacist will review medication regimens, address pharmacy referrals, and collaborate with clinical teams to optimize patient care in a remote setting.
Key responsibilities
- Review medication regimens and provide recommendations based on current protocols and guidelines
- Create written responses to pharmacy referrals regarding medication access, education, and safety
- Collaborate with care providers to enhance medication management and participate in quality improvement activities
Required qualifications
- Active Pharmacist License
- PharmD degree from an accredited School of Pharmacy
- 5 years of clinical pharmacy experience in direct collaboration with healthcare providers
- Experience in renal patient pharmacy management and value-based care preferred
- Understanding of CMS Star measures and HEDIS measures
