Job Summary
To support a growing patient consultation service, the part-time State Licensed Pharmacist will conduct telephonic clinical consultations, perform patient assessments, and provide medication education while working remotely.
Key responsibilities
- Conducts telephonic clinical consultations to support safe and effective use of specialty medications
- Performs patient assessments and medication reconciliation to identify adherence barriers and medication-related concerns
- Provides education on disease state management, medication administration, and side effect mitigation strategies
Required qualifications
- Graduate of an ACPE Accredited School of Pharmacy with a bachelor's degree in Pharmacy or PharmD
- Current, unrestricted licensure as a Registered Pharmacist in the state of residence
- Experience counseling patients and caregivers regarding medication therapy and disease state management
- Experience documenting patient assessments and clinical interventions in accordance with pharmacy practice standards
- Experience evaluating medication regimens to identify potential drug interactions and adherence barriers
