Job Summary
Seeking a State Licensed Transitions of Care Physician, the full-time remote role focuses on delivering high-quality virtual patient care to recently discharged patients at risk for readmission, while also providing clinical supervision and leadership to an interdisciplinary team.
Key responsibilities:
- Deliver high-quality direct virtual patient care to manage complex transitions of care cases
- Serve as the final escalation point for clinical questions and oversee clinical outcomes to improve readmission rates
- Provide supervision and coaching to Nurse Practitioners and lead interdisciplinary rounds for high-risk patients
Required qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certification in Internal Medicine, Family Medicine, or related specialty
- Minimum 3 years of post-residency clinical experience
- Strong background in inpatient care and transitions of care
- Experience leading interdisciplinary rounds and supervising healthcare professionals
