Job Summary
Providing holistic, evidenced-based care, the full-time State Licensed Virtual Care Nurse Practitioner will deliver patient services remotely, including assessment, diagnosis, treatment, and health counseling for patients aged 18 months and above, with evening shifts from 2:30 p.m. to 11:00 p.m. and every other weekend required.
Key responsibilities:
- Deliver comprehensive patient care through accurate assessment, diagnosis, treatment, and management of health problems
- Evaluate and provide healthcare education for primary care and transitional care patients, ensuring effective disposition planning
- Document patient care in the EHR and collaborate with internal and external colleagues to maintain high standards of care
Required qualifications:
- Current, unrestricted license in D.C., DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, or VT
- Minimum of two years of medically relevant experience or equivalent
- Master's Degree in Family Nurse Practitioner program with current National Board Certification
- Ability to work autonomously and effectively within a patient care team
- Proficiency in information management and technology; bilingual skills may be required based on market needs
