Substance Use Disorder Care Coordinator

Job Summary

To enhance care for individuals with substance use disorders, the full-time Substance Use Disorder Care Coordinator will manage client assessments, coordinate treatment plans, and facilitate access to resources in a remote setting.

Key responsibilities

• Conduct comprehensive assessments to determine client needs and treatment options
• Coordinate and implement individualized care plans in collaboration with healthcare providers
• Facilitate access to community resources and support services for clients and their families

Required qualifications

• Bachelor's degree in social work, psychology, or a related field
• Experience in substance use disorder treatment or care coordination
• Knowledge of community resources and healthcare systems
• Certification in substance use disorder counseling or related credentials preferred
• Ability to work independently and manage a caseload effectively
