Job Summary

A major healthcare company needs applicants for an opening for a Telecommute Director of Population Health Management Consulting.

Candidates will be responsible for the following:

• Defining the business development strategy for a subset of the practice
• Business Development
• Owning and managing sales accounts, leads, pursuits, and proposals

Required Skills:

• 5 years ofexperience in a number of population health management programs / functions
• 5 years of experience across population health management capabilities
• 5 years of team lead, consulting leadership, or project management experience
• Management, Transition Management, Wellness, etc.
• Experience developing new business and identifying add-on work
• Understanding of provider-embedded and led population health models
