Telehealth Nurse Practitioner
Job Summary
To support a growing care navigation program, the Telehealth Nurse Practitioner will conduct remote video visits for follow-up and chronic condition management, serve as a clinical resource for the care team, and assist with care transitions for members, available in both part-time and full-time capacities.
Key responsibilities
- Conduct telehealth visits for chronic condition check-ins and medication reconciliation
- Support members post-hospital discharge by completing follow-up visits and coordinating care
- Act as a clinical resource for the care team, providing training and responding to clinical escalations
Required qualifications
- Board certification as a Family (FNP) or Adult-Gerontology (AGNP) Nurse Practitioner
- Active, unrestricted APRN license in at least one U.S. state
- 3+ years of NP experience in primary care, internal medicine, geriatrics, or transitional care
- Proficient with telehealth platforms and electronic health records (EHRs)
- Availability during core business hours, Monday-Friday, Eastern Time
