Job Summary
Focusing on improving members' health and wellness via tele-pharmacy, the full-time Telehealth RN Case Manager will provide clinical outreach, develop clinical resources, and support pharmacy staff through remote engagement with members and prescribers.
Key responsibilities
- Develop and update clinical algorithms, pathways, and call scripts to support pharmacy staff
- Provide telehealth coaching and clinical outreach to members and prescribers to enhance care coordination
- Collaborate with department staff to maintain high quality and consistent clinical programs
Required qualifications
- Associate's degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required; BSN preferred
- Current, active Registered Nurse (RN) license in good standing
- 2+ years of clinical experience, including direct consultation experience
- Experience in a clinical call center is preferred
- Knowledge of healthcare industry practices and HIPAA regulations
