Job Summary
To support an innovative telehealth program, the full-time Diabetes Telemedicine Nurse Practitioner will manage a structured 14-week diabetes management program, conducting weekly virtual visits, adjusting medications, and developing personalized care plans for patients across the United States.
Key responsibilities
- Conduct weekly follow-up visits and provide education to patients living with diabetes
- Assess, diagnose, and treat individuals newly diagnosed with diabetes or those needing intensive management
- Collaborate with a multidisciplinary team to deliver coordinated, patient-centered care
Required qualifications
- Minimum of 1-2 years of experience in diabetes management as an advanced practice provider
- Experience with continuous glucose monitoring (CGM), insulin pumps, and automated insulin delivery (AID) systems is highly desirable
- CDCES and/or BC-ADM certification preferred but not required
- Strong clinical judgment and critical thinking skills
- Exceptional interpersonal and telehealth communication skills
