Tennessee Licensed Care Manager

Job Summary

Overseeing daily operations of the care coordination team, the full-time Remote Manager, Continuum of Care will manage care transition programs, evaluate processes, and collaborate with post-acute network partners to enhance care alignment and reduce readmissions.

Key responsibilities

• Manages day-to-day operations and workflows of care coordination teams to ensure compliance and effective care delivery
• Directs staff management activities including recruitment, performance evaluations, and professional development
• Evaluates care transition processes to identify efficiencies and improve patient outcomes in alignment with value-based care objectives

Required qualifications

• Bachelor's Degree in Nursing, Social Work, Healthcare Administration, or a related field required
• 5-7 years of healthcare experience in clinical care, care coordination, or healthcare operations required
• 2-4 years of supervisory or leadership experience required
• Experience within Accountable Care Organizations (ACOs) or value-based care delivery models preferred
• RN or LCSW licensure preferred; CCM certification is a plus
