Job Summary
To support a growing hospice and home health team, the part-time Tennessee Licensed LPN Nurse will manage nurse visit encounters, prioritize based on acuity, and coordinate with on-call staff while working remotely every other weekend from 10 AM to 7 PM CST.
Key responsibilities
- Manage encounters that require a nurse visit and are placed in a workgroup by a triage nurse
- Prioritize workgroup encounters based on acuity and utilize internal tools to identify the correct on-call staff member
- Place outbound calls to arrange visits and relay call information to on-call staff
Required qualifications
- MUST have or be willing to obtain a Compact LPN license
- Experience with end-of-life care is required
- Bilingual in English and Spanish, with proficiency in speaking, writing, and reading
- Must have high-speed internet and be tech-savvy
- Ability to multitask in a fast-paced environment
