Job Summary
To support TennCare members in accessing long-term services and supports, the full-time remote Tennessee Licensed Member Support Coordinator will manage telephonic screenings, handle incoming referrals, and educate members about available services while ensuring timely progression through the screening process.
Key responsibilities
- Serve as a primary contact for members and family representatives, assisting with the CHOICES program and navigating the referral process
- Conduct telephonic screenings and manage requests related to Long Term Services and Support (LTSS), including referrals and enrollment coordination
- Collaborate with internal departments and external partners to resolve service delivery concerns and ensure compliance with requests
Required qualifications
- Master's in Social Work (LCSW, LMSW, or LAPSW), Registered Nurse (RN), or Licensed Practical Nurse (LPN) license
- At least 3 years of clinical experience required
- Proficiency in Microsoft Office (Outlook, Word, Excel, and PowerPoint)
- Exceptional time management and organizational skills
- Ability to work independently and manage multiple priorities in a fast-paced environment
