Job Summary
To support a leading national healthcare organization, the part-time contract Tennessee Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and enhance patient education while working remotely from Tennessee.
Key Responsibilities
- Conduct telephonic consultations to complete comprehensive medication reviews and adherence assessments
- Identify gaps in care, high-risk medications, and therapeutic alternatives while evaluating medication regimens
- Provide patient education on medications and treatment plans, and communicate clinical recommendations to members and healthcare providers
Required Qualifications
- Active, unrestricted Pharmacist license in Tennessee
- Strong understanding of disease-state management and the core elements of Medication Therapy Management (MTM)
- Minimum 1 year of experience working remotely or independently in a collaborative team environment
- Ability to navigate multiple clinical platforms and applications simultaneously
- Flexibility to work rotating shifts between 9:00 AM and 8:00 PM CST, Monday-Friday
