Tennessee Licensed RN Case Manager

Job Summary

Seeking a Tennessee Licensed RN Case Manager, the full-time remote position will manage complex healthcare needs for members with chronic conditions, collaborating with multidisciplinary teams to enhance care coordination and improve health outcomes.

Key responsibilities

• Support utilization management functions for complex cases and serve as a liaison between members and providers
• Oversee implementation of interventions for highly complex cases to ensure efficient utilization of benefits
• Perform essential case management activities, including assessment, planning, implementation, and evaluation

Required qualifications

• Active Registered Nurse (RN) license in Tennessee or a participating Nurse Licensure Compact state
• Minimum of 3 years of clinical experience and 5 years in the healthcare industry
• Experience in acute care settings such as ED, ICU, or Med-Surg
• Background in case management, transitions of care, or discharge planning
• Strong technical proficiency and digital literacy for effective communication and care delivery
