Job Summary
To support a patient-centered specialty pharmacy team focused on home infusion care, the remote Texas Licensed Care Manager (RN) will coordinate home infusion nursing services, monitor clinical data, and provide patient education from Monday to Friday, 9:00 AM to 5:30 PM EST.
Key responsibilities
- Coordinate home infusion nursing services through contracted providers and address patient concerns
- Monitor clinical data, assess patient needs, and conduct patient outreach for safe home infusion therapy
- Collaborate with healthcare providers and internal departments to promote positive patient outcomes
Required qualifications
- Degree from an accredited nursing program or BSN
- Active RN license in Texas or Florida, or an applicable compact state license
- At least 2 years of clinical nursing experience in clinical, acute, community health, or home health settings
- Home infusion experience including familiarity with care coordination and patient education
