Texas Licensed Care Navigator

Job Summary

To support families affected by chronic conditions, the part-time Texas Licensed Care Navigator will provide virtual biopsychosocial support, personalized care planning, and community resource connections while collaborating with a transdisciplinary clinical team.

Key responsibilities

• Act as the primary point of contact for caregivers and families, enhancing quality of life and safety in the home
• Conduct biopsychosocial assessments to identify strengths, goals, and unmet health-related needs
• Develop personalized care plans and provide disease-specific education while ensuring continuity of care with healthcare specialists

Required qualifications

• Two years of experience in a healthcare or community-based setting
• One year of experience understanding the needs of family caregivers and older adults
• Demonstrated cultural humility and ability to interact with diverse populations
• Familiarity with Electronic Health Records (EHRs) is a plus
• Proficiency in another language, other than English, is a plus but not required
