Job Summary
Working remotely on a full-time basis, the Texas Licensed Clinical Coordinator RN will manage the review and coordination of Minor Home Modifications and Vehicle Modifications for HCBS Waiver Service Coordinators, while collaborating with vendors and conducting interdisciplinary meetings to ensure quality care.
Key responsibilities:
- Review and coordinate requests for Minor Home Modifications and Vehicle Modifications, ensuring compliance with health and safety standards
- Facilitate interdisciplinary meetings and collaborate with service coordinators and management to enhance operational efficiency
- Track data trends and identify training opportunities to improve service delivery and operational practices
Required qualifications:
- Current unrestricted Texas RN license
- 2+ years of experience working in Medicaid and the HCBS Star Plus Waiver program
- 2+ years of experience with the Minor Home Modification and ISP development process
- Advanced proficiency in the Star Plus Waiver Benefit structure
- High-speed internet access at residence
