Texas Licensed Clinical Pharmacist

Job Summary

To enhance patient care, the part-time Texas Licensed Clinical Pharmacist will provide integrated support to patients and physicians by developing personalized relationships, reviewing clinical data for care plans, and monitoring medication adherence remotely.

Key responsibilities

• Review and interpret clinician's notes to support prior authorization approvals and develop care plan recommendations
• Monitor patient adherence across various disease categories, focusing on oncology and specialty diseases
• Conduct medication therapy management, including optimizing drug regimens and advising clinicians on evidence-based therapy

Required qualifications

• Doctor of Pharmacy (PharmD) or equivalent advanced degree required
• Must be a licensed Pharmacist in Texas
• 1+ years of experience in a clinical pharmacy setting preferred
• Proficiency in Microsoft Office tools for communication and data management
• Knowledge of complex/chronic disease states and therapies used within specialty pharmacy preferred
