Job Summary
To support a new Longevity Program, the remote Texas Licensed Geriatric Nurse Practitioner will deliver initial assessments and ongoing medical follow-ups to older adults via telehealth, focusing on comprehensive geriatric assessment, chronic disease management, and cognitive health.
Key responsibilities
- Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk
- Provide medical follow-up visits for chronic disease management and ongoing care
- Perform cognitive screening and staging, escalating for further assessment as needed
Required qualifications
- Active, unrestricted NP license in Texas
- Board certification as an Adult-Gerontology NP or Family NP with a geriatric focus
- Active DEA registration
- Experience with Medicare documentation and E/M coding
- Comfort with telehealth and EHR documentation
