Texas Licensed MTM Pharmacist

Job Summary

To support a growing clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document care delivered while working remotely with flexible hours between 8:00am and 8:00pm CST.

Key responsibilities

• Engage members telephonically to schedule consultations and provide medication therapy management
• Assess medication regimens for appropriateness and educate patients on their medication and disease states
• Document patient care notes clearly and participate in departmental meetings to enhance clinical programs

Required qualifications

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of remote work experience in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Ability to work a flexible schedule with rotating shifts as needed
