Texas Licensed MTM Pharmacist
Job Summary
To support a growing Clinical Engagement Services Center, the contract-to-potential-FTE Texas Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely with a flexible schedule.
Key responsibilities
- Engage members telephonically to schedule consultations and provide clinical recommendations
- Prioritize patient privacy and adhere to HIPAA regulations while documenting care delivered
- Assess medication regimens for effectiveness and educate patients on their medications and disease states
Required qualifications
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and MTM services
- 1+ years of experience working remotely and independently in a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
