Texas Licensed MTM Pharmacist
Job Summary
To support a dynamic clinical team, the contract MTM Pharmacist will engage in telephonic consultations for comprehensive medication reviews and disease state management while working remotely in Texas with flexible hours between 8:00am and 8:00pm CST.
Key responsibilities:
- Conduct telephonic consultations and schedule appointments for members in clinical programs
- Assess medication regimens and provide clinical recommendations based on MTM protocols
- Document patient care notes and participate in departmental meetings to enhance clinical services
Required qualifications:
- Current and unrestricted Pharmacist license in Texas
- Strong knowledge of disease state management and the 5 Core Elements of MTM Services
- 1+ years of remote work experience in a cross-functional team environment
- Proficiency in Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
