Texas Licensed MTM Pharmacist
Job Summary
To support a growing clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide disease state management while working remotely in Texas with flexible hours between 8:00am and 8:00pm CST.
Key responsibilities
- Engage members telephonically and schedule consultations for clinical programs
- Utilize prescription claims data to generate clinical recommendations and assess medication regimens
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications
- Current and unrestricted Pharmacist license in Texas
- Strong knowledge of disease state management and the 5 Core Elements of MTM Services
- 1+ years of remote work experience in a cross-functional team environment
- Proficiency in navigating MTM platforms and using Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D required
