Texas Licensed RN Case Manager
Job Summary
To support a diverse member population, the full-time Texas Licensed RN Case Manager will work remotely to assess health statuses, identify treatment barriers, and provide patient education and resource connections.
Key responsibilities
- Make outbound and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans and provide patient education for self-management
- Facilitate referrals to outside resources and maintain communication with members for quality care
Required qualifications
- Current, unrestricted RN license in Texas or a Compact RN license
- 10+ years of experience in case management, hospital, acute care, or direct care settings
- Ability to type and navigate a Windows-based environment
