Job Summary
To support members with physical health conditions and chronic disease management, the full-time Texas Licensed Senior Care Manager RN will assess, plan, and implement complex care management activities while working remotely and collaborating with healthcare providers to ensure high-quality care outcomes.
Key responsibilities
- Develop and continuously assess personalized care plans, collaborating with providers to address members' needs
- Coordinate care between members, families, and care providers to ensure appropriate person-centered services
- Monitor member status and care plans, revising as necessary to meet goals and address any barriers to care
Required qualifications
- Degree from an accredited school of nursing or a Bachelor's degree in nursing
- 4-6 years of related experience in clinical nursing with direct patient care
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Preferred 7+ years of clinical nursing experience, particularly with adult populations
- Experience with state-government sponsored programs (Medicaid, STAR CHIP) is a plus
