Job Summary
To support the Texas Child Health Access Through Telemedicine (TCHATT) program, a Texas Licensed Senior Social Worker (LMSW/LCSW) will provide remote social work services focused on assessing and addressing the medical, psychiatric, and social needs of students, while collaborating with multidisciplinary teams and community resources.
Key responsibilities:
- Conduct comprehensive assessments to interpret patients' social, emotional, environmental, and financial needs
- Plan treatment courses or refer cases to appropriate professionals to ensure patients receive suitable care
- Collaborate with physicians and healthcare providers to develop and monitor individualized care plans
Required qualifications:
- Bachelor's Degree in Social Work or related field required; Master's Degree in Social Work preferred
- 1 year of social work experience required; medical social work experience preferred
- Licensed Baccalaureate Social Worker (LBSW) by the State of Texas required
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) by the State of Texas preferred
- Experience delivering evidence-based treatment modalities specific to trauma is preferred
