Job Summary
Providing support and guidance to members with chronic conditions, the full-time Texas Licensed Wellness Coach will conduct health assessments, implement care plans, and offer telephonic coaching to improve health and wellness outcomes in a remote setting.
Key responsibilities
- Utilize clinical expertise to provide health support and solutions for members with chronic health conditions
- Perform telephonic assessments to evaluate member needs and readiness to change
- Engage members in developing wellness goals and monitor their progress to ensure effective care plans
Required qualifications
- Associate's degree or higher; Master's degree in Mental Health, Counseling, Social Work, or Nursing preferred
- 2-4 years of related behavioral health or psychiatric experience
- Current Texas state licensure as an LPC, LCSW, LMSW, LMFT, or RN required
- Experience in psychiatric intensive outpatient programs or managed care behavioral health management
- Proficient in Microsoft Office applications; bilingual in English and Spanish is a plus
