Job Summary
To support members with chronic conditions, the full-time Texas Licensed Wellness Health Coach will conduct health assessments, implement care plans, and provide telephonic support and education in a remote setting.
Key responsibilities:
- Utilize clinical training to deliver health support and solutions to members
- Perform telephonic assessments to evaluate member needs and readiness for change
- Engage members in developing and monitoring wellness goals and plans
Required qualifications:
- Associate's degree or higher; Master's degree in Mental Health, Counseling, or Social Work preferred
- 2-4 years of related behavioral health or psychiatric experience
- Current Texas state licensure as LPC, LCSW, LMSW, LMFT, or RN required
- Experience in psychiatric intensive outpatient programs or managed care behavioral health
- Proficiency in Microsoft Office applications
