Transfer & Referral Specialist

Job Summary

To support patient care coordination, the full-time Transfer & Referral Specialist will review clinical information, facilitate referrals to appropriate specialists, and maintain documentation and communication with healthcare providers in a remote capacity.

Key responsibilities:

• Reviews clinical information to identify appropriate services and facilitates patient referrals based on payer rules and specialist capacity
• Documents referral requests and interactions in the database, ensuring accurate follow-up tracking and auditing
• Communicates with physicians and case management to ensure timely follow-up and maintains current knowledge of available clinical services

Required qualifications:

• Bachelor's degree in Business, Healthcare Administration, or a related field, or equivalent experience
• Two years of recent relevant experience in a healthcare setting
• Working knowledge of managed care processes and insurance authorization
• Familiarity with medical terminology and HIPAA requirements
• Ability to handle confidential information discreetly and effectively
