Transitional Care Navigator

Job Summary

Working remotely, the full-time Transitional Care Navigator will manage initial customer contacts, ensuring timely and accurate processing of orders while collaborating with the sales team to provide exceptional customer service.

Key responsibilities:

• Ensure positive interactions with internal and external customers while maintaining confidentiality and adhering to documentation standards
• Collaborate with sales team members to facilitate smooth transitions of care and accurately assess customer needs
• Process referrals and verify insurance benefits, resolving customer concerns and complaints effectively

Required qualifications:

• Associates degree or equivalent experience required
• Exceptional phone and customer relations skills
• Healthcare experience and knowledge of medical terminology preferred
• Previous experience with a medical billing system preferred
• Ability to multitask and build relationships within a team environment
