Job Summary
Supporting the planning and implementation of the Utah Community Care Hub, the full-time salaried Improvement Advisor will collaborate with health care providers, local health departments, and community organizations to enhance care coordination and health outcomes.
Key responsibilities
- Guide organizations through quality improvement and community health initiatives
- Design and implement evidence-based quality improvement interventions and support care coordination workflows
- Facilitate trainings, technical assistance, and partner engagement meetings
Required qualifications
- Bachelor's degree in public health, health care, or a related field
- 4+ years of experience in health care, public health, or community-based social services, including quality improvement work
- 2+ years of project management experience
- Strong knowledge of quality improvement methods and clinical workflows
- Excellent communication and partner engagement skills
