Utah Licensed Clinical Care Coordinator

Job Summary

Supporting a growing telehealth and partner clinic network, the full-time salaried Utah Licensed Clinical Care Coordinator will manage patient communications, coordinate care, and collaborate with providers in a remote environment.

Key responsibilities

• Communicate with patients through various channels to provide a compassionate, patient-centered experience
• Coordinate prescription workflows and collaborate with internal teams to support patient care
• Document patient interactions accurately and adapt to evolving clinical programs and workflows

Required qualifications

• Active LVN/LPN license or relevant medical office experience
• Minimum of 2 years of clinical or medical experience within the last 4 years
• At least 1-2 years of remote or telehealth experience
• Strong organizational skills and ability to prioritize tasks in a fast-paced environment
• Proficient with computers and capable of quickly learning new software and workflows
