Job Summary
To support patient care initiatives, the part-time Utah Licensed Clinical Pharmacist will manage medication refill authorizations remotely, ensuring timely access to prescribed medications while collaborating with primary care clinics and contributing to quality improvement efforts.
Key responsibilities
- Reviews patients' medication orders for safety, appropriateness, and compliance, making evidence-based interventions as needed
- Ensures timely preparation and delivery of medications, maintaining regulatory compliance and safety standards
- Identifies areas for process improvement and participates in departmental initiatives to enhance patient satisfaction and safety
Required qualifications
- Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution
- Active pharmacist license in Utah, Colorado, Montana, or Nevada
- Current knowledge of state and federal pharmacy regulations
- Immunization certification if applicable to practice site needs
- Previous experience in an integrated healthcare delivery system is preferred
