Job Summary
Working remotely on a full-time basis, the Utah Licensed RN Care Manager will manage care for members with chronic conditions, collaborating with healthcare teams to develop and implement care plans while ensuring access to necessary resources and services.
Key responsibilities
- Collaborate with physicians and healthcare teams to improve patient health and manage chronic conditions
- Develop, document, and implement individualized care plans addressing members' comprehensive needs
- Coordinate communication among members, families, and healthcare providers to ensure effective care management
Required qualifications
- Current Utah RN license or eligibility to obtain within 60 days of hire
- Minimum of 1-year experience in care management or a closely related field
- Demonstrated clinical nursing experience
- Intermediate computer skills and ability to troubleshoot software issues
- RNs must obtain a BSN within three years of hire or promotion
