Utah Licensed RN Care Manager
Job Summary
Working remotely on a full-time basis, the Utah Licensed RN Care Manager will manage a portfolio of members, collaborating with healthcare teams to assess, plan, and coordinate care that meets the comprehensive medical, behavioral, and social needs of patients while promoting self-management and quality outcomes.
Key responsibilities
- Develop and implement individualized care plans addressing members' diverse needs
- Facilitate communication and coordination among members, families, and healthcare teams
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes
Required qualifications
- Current RN license in Utah or ability to obtain within 60 days of hire
- Minimum of 1 year of experience in care management or a related field
- Demonstrated clinical nursing experience
- Intermediate computer skills and ability to troubleshoot software issues
- RNs must obtain a BSN within three years of hire or promotion
