Utah Licensed RN Case Manager

Job Summary

To support a growing healthcare team, the part-time Utah Licensed RN Case Manager will provide telephonic proactive case management services to members with frequent Emergency Department utilization, focusing on reducing avoidable visits and hospital admissions through care coordination and patient engagement.

Key responsibilities

• Deploy RN case management support for high-risk and frequent ED utilizers within assigned markets
• Conduct comprehensive assessments to identify clinical, psychosocial, and social determinants of health needs
• Deliver proactive outreach and care coordination to promote self-management and adherence to care plans

Required qualifications

• Current, unrestricted Registered Nurse (RN) license in Utah
• 3+ years of clinical nursing experience, preferably in acute care or case management
• Experience in managed care, Medicare Advantage, or population health environments
• Demonstrated ability to independently assess member needs and develop care plans
• Proficiency with electronic medical records and case management systems
