Vermont Licensed MTM Pharmacist
Job Summary
To support a dynamic Clinical Engagement Services Center, the remote Vermont Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and ensure adherence to clinical guidelines while working on a contract basis with potential for full-time employment.
Key responsibilities:
- Engage members telephonically to schedule and conduct pharmacist consultation appointments
- Utilize prescription claims data to provide clinical recommendations and assess medication regimens
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications:
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and MTM services
- 1+ years of experience working remotely within a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
