Vermont Licensed MTM Pharmacist

Job Summary

To support a growing clinical engagement team, the part-time contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and improve disease state management while working remotely in Vermont.

Key responsibilities

• Engage members telephonically to schedule consultations and provide medication therapy management services
• Document patient care notes and communicate essential information to primary care providers while adhering to HIPAA regulations
• Utilize prescription claims data to generate clinical recommendations and educate patients on their medication regimens

Required qualifications

• Current and unrestricted Pharmacist license in Vermont
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely within a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Bachelor's Degree or Pharm D
