Job Summary
Providing comprehensive clinical services, the part-time Virginia Licensed Call Center Nurse will coordinate telephonic care for patients, focusing on transitions of care, medication adherence, and managing gaps in care while working remotely in Chesapeake, VA.
Key responsibilities:
- Coordinate with healthcare providers and patients to deliver nursing support and manage care processes
- Assist patients in self-management and behavior modifications to improve health outcomes
- Educate patients on best practices and standard workflows while ensuring high-quality service delivery
Required qualifications:
- An associate nursing degree or equivalent clinical experience
- Current, unrestricted Virginia RN License
- 3+ years of experience in an Acute or Managed Care setting
- Experience with Electronic Medical Records system(s)
- Demonstrated knowledge of chronic illnesses and medications used for treatment
