Virtual Care Coordinator

Job Summary

Supporting patients and clinicians nationwide, the full-time Virtual Care Coordinator will manage care coordination for health coaching and nutrition services, triage non-clinical patient needs, and ensure a seamless care experience in a fully remote environment.

Key responsibilities

• Partner with cross-functional teams to coordinate patients' health coaching and nutrition journeys
• Outreach to patients for navigation of care journeys, including insurance, service options, and appointment scheduling
• Manage inbound patient messages and provide real-time administrative support during virtual visits

Required qualifications

• High school diploma or equivalent
• 2+ years of experience in a virtual care, patient coordination, or Medical Assistant role
• Familiarity with virtual care workflows in a primary care or chronic care setting
• Prior experience supporting care in a multi-state, remote care delivery model
• Comfort with remote tools and EMR platforms, preferably Athena
