Job Summary
To support families affected by chronic conditions and serious illness related to aging, the part-time Washington Licensed Care Navigator will provide virtual biopsychosocial support, personalized care planning, and community resource connections while collaborating with a transdisciplinary clinical team.
Key responsibilities
- Act as the primary point of contact for caregivers and families, enhancing their quality of life and safety at home
- Conduct biopsychosocial assessments to identify needs and develop personalized care plans
- Communicate with healthcare specialists to ensure continuity of care and provide caregiver education
Required qualifications
- Two years of experience in a healthcare or community-based setting preferred
- One year of experience understanding the needs of family caregivers and older adults
- Familiarity with Electronic Health Records (EHRs) is a plus
- Strong computer skills and ability to troubleshoot technology issues
- Proficiency in another language, other than English, is a plus but not required
