Job Summary
Transforming home-based care, the full-time Washington Licensed Nurse Practitioner will conduct remote assessments for home health patients, act on predictive health signals in real time, and coach care teams through virtual touchpoints while working in a hybrid model, spending three days a week with patients in their homes and two days a week delivering care via telehealth.
Key responsibilities
- Conduct initial and follow-up telehealth assessments on flagged patients, adjusting plans of care as necessary
- Interpret trend-based insights from the Circadia platform to prioritize high-risk patients and drive timely interventions
- Efficiently complete 25+ tele visits per day while ensuring documentation is completed within the same day
Required qualifications
- Certified and licensed Nurse Practitioner in Washington with ANCC (or equivalent) in Adult/Family/Geriatric/Acute
- 2+ years of NP experience with complex adult populations in home health or post-acute settings
- Comfortable conducting telehealth visits and navigating virtual care platforms
- Eligible for Medicare participation
