Job Summary

To enhance patient care in a home health setting, a Washington Licensed Physical Therapist will conduct assessments, develop treatment plans, and provide interventions to improve patients' functional status and well-being in a remote capacity.

Key responsibilities:

• Evaluates patients' functional status and collaborates with physicians to develop therapy plans
• Implements treatment interventions and monitors patient progress, reporting changes to the healthcare team
• Instructs patients and caregivers on the plan of care and maintains accurate documentation of visits and evaluations

Required qualifications:

• Current Physical Therapy licensure in the state of Washington
• Current driver's license and vehicle insurance, with access to a dependable vehicle or public transportation
