Washington Licensed Social Worker

Job Summary

To support a growing multidisciplinary care team, the full-time Washington Licensed Social Worker will provide remote behavioral health assessments, brief therapy, crisis intervention, and care coordination for patients with complex needs.

Key responsibilities

• Support the behavioral health needs of an assigned patient caseload using evidence-based models of care
• Collaborate with prescribing providers on psychotropic medication management and treatment adherence
• Document patient encounters and treatment plans in the electronic health record accurately and timely

Required qualifications

• Master's degree in Social Work
• Current Washington independent licensure required
• 2+ years of prior healthcare or behavioral health experience
• Geriatric care experience highly desired
• Proficient in Microsoft Office Suite and electronic medical records (EMR) systems
