Washington State Licensed Case Manager, RN

Job Summary

To support a growing case management team, the full-time telecommuting Washington State Licensed Case Manager, RN will engage members facing medical utilization disorders, conduct holistic health assessments, and facilitate the development and monitoring of member-centric case management plans.

Key responsibilities

• Engage members in open discussions about sensitive health topics and support them in overcoming barriers to care
• Conduct initial holistic health assessments to identify social determinants of health and collaborate on personalized care plans
• Monitor and evaluate the effectiveness of case management plans, ensuring goals remain relevant and progress is tracked

Required qualifications

• Active Washington State or Compact RN or ARNP license for independent assessments
• Three years of post-licensure direct care experience
• One year of post-licensure case management experience involving care coordination or management of patient care
• Experience with motivational interviewing techniques
• Ability to maintain accurate and timely documentation meeting accreditation and legal requirements
