Job Summary
Providing nursing care remotely, the full-time Washington State Licensed RN will assess patient needs, develop care plans, and coordinate health services for a defined group of patients in collaboration with the healthcare team.
Key responsibilities:
- Conduct ongoing assessments and develop individualized care plans based on patient needs
- Provide telephone consultations and education to patients and their families regarding health care goals and interventions
- Collaborate with healthcare team members to ensure comprehensive patient care and modify care plans as needed
Required qualifications:
- Associates degree in Nursing from an accredited program
- Registered Nurse License (Washington) or Compact License required at hire
- Basic Life Support certification required at hire
- Ability to demonstrate effective communication and decision-making skills
- Participation in continued education programs related to nursing practice
