Job Summary

Providing nursing care remotely, the full-time Washington State Licensed RN will assess patient needs, develop care plans, and coordinate health services for patients, while working within the guidelines of health care standards and policies.

Key responsibilities

• Conduct ongoing assessments of patient care needs and develop individualized care plans
• Provide telephone consultations and education to patients and their families regarding health care goals and interventions
• Collaborate with health care team members to ensure comprehensive care and effective communication

Required qualifications

• Associates degree in Nursing from an accredited program
• Registered Nurse License (Washington) or Compact License required at hire
• Basic Life Support certification required at hire
• Experience in ambulatory care or case management is preferred
• Bachelor of Science in Nursing (BSN) is preferred
