Job Summary

Providing nursing care to a defined group of patients, the full-time salaried Washington State Licensed RN will work remotely, coordinating health care services and collaborating with the health care team to develop and implement individualized care plans.

Key responsibilities

• Conduct ongoing assessments and develop written plans of care based on patient needs
• Provide telephone consultations and education to patients and their families regarding health care goals and interventions
• Collaborate with health care team members to modify care plans and ensure quality of care

Required qualifications

• Associates degree in Nursing from an accredited program
• Registered Nurse License (Washington) or Compact License required at hire
• Basic Life Support certification required at hire
• Experience in ambulatory care nursing is preferred
• Bachelor of Science in Nursing (BSN) is preferred
